    I think you are referring to one of the promo pics from “Recoil” season 5, episode 16. In the pic she’s wearing a white sleeveless shirt and there’s blood on the shirt. Gibbs is checking to make sure she’s ok.
    In the episode Ziva had been working undercover tracking a serial killer and he got suspicious of her. She had to defend herself and ended up shooting him. He fell dead on top of her and that is how the blood got on her clothes.

    they have pics and episode recaps for NCIS along with hundreds of other tv shows. hope that helps.
